I/we acknowledge that these records will be held and used by St Matthew’s Collegiate and any other organisation or group that functions under the auspices of the School for any lawful activity that those bodies undertake and that the information may be provided to education authorities if required by law.
I/We acknowledge that the information we have provided will be used to determine whether or not the applicant is offered an interview for a place at St Matthew’s Collegiate.
I/We are aware of our rights to have access to the information, to request correction of it, to be informed of any action taken in response to such a request and to have attached to the information a statement that we have requested a correction.
I/We authorise St Matthew’s Collegiate to obtain any information about the applicant that may be necessary to achieve the purpose of educating the School’s pupils and to disclose any information obtained during the course of
the applicant’s education to the applicant’s parent/guardian(s).
I/We acknowledge that any acceptance of an offer of a place at St Matthew’s Collegiate requires that parents and their daughter agree to adhere to the Term and Conditions of Admission attached to and forming part of the Admission Agreement.
I/We agree to notify St Matthew’s Collegiate of any change in the information contained in this application as soon as is reasonably practical.
I/We agree to forward the sum of $250 at the time of acceptance. Disbursement expenses are accumulated by the School and invoiced to parent/guardian (s) periodically during the year. This is a voluntary disbursement deposit of $250 requested by the Board prior to the student commencing at the School. This deposit is refunded to parents when the student leaves the School after all disbursements have been paid.
